Emergency medicine evaluation and management coding uses a dedicated set of codes (99281-99285) that reflect the complexity of the patient presentation and the resources consumed during the visit. The system reads the clinical note and identifies each problem addressed, categorizing them by complexity (self-limited, low severity, moderate severity, high severity). It identifies data elements documented: labs reviewed, imaging reviewed, external records obtained, independent interpretation of studies. It evaluates the risk based on the documented management decisions: prescription drug management, decision to observe, minor procedures, emergency procedures, and decisions regarding hospitalization.

Based on this analysis, the system determines the MDM level that the documentation supports and compares it to the level coded by the provider. When there is a mismatch, the system flags the encounter for review.

Documentation Improvement Prompts

The most valuable intervention happens when the AI identifies that the clinical scenario supports a higher level than the documentation captures. In emergency medicine, this is common. A physician manages a patient with chest pain, reviews an ECG and troponin levels, considers and rules out acute coronary syndrome, and discharges the patient. The clinical work clearly supports a level 4 or 5 visit, but if the note does not document the data reviewed or the differential considered, the documented MDM might only support level 3.

AI systems prompt the provider to complete their documentation before the note is finalized. The prompt is specific: your note describes managing a patient with chest pain but does not document review of the ECG findings or the differential diagnosis considered. Adding this documentation would support a level 4 E/M code. This targeted feedback helps providers document what they actually did rather than leaving billable work undocumented.

Procedure and Critical Care Capture

Emergency medicine involves significant procedural work (laceration repair, fracture reduction, lumbar puncture, central line placement) and critical care time that are separately billable beyond the base E/M service. AI systems check the ED note for documented procedures and critical care time that might not have been captured in the charge entry.

When the note describes a procedure but no corresponding procedure charge exists, the system flags the gap. When the note documents critical care time (which requires specific documentation of time spent in direct management of a critically ill patient), the system calculates the billable critical care units and verifies that the documentation supports the time claimed.

Observation and Admission Decision Coding

ED encounters that result in observation or inpatient admission have different coding and billing rules than those that result in discharge. The decision to observe or admit generates additional billing opportunity (observation care codes or initial hospital care codes) but requires specific documentation. AI systems identify encounters that result in observation or admission and verify that the documentation supports the appropriate coding for both the ED visit and the subsequent observation or admission.

Payer-Specific ED Billing Rules

Some payers have specific ED billing policies that differ from standard Medicare rules. Some commercial payers downcode ED visits automatically based on the final diagnosis regardless of the documented complexity. Some Medicaid programs have their own ED visit level criteria. AI systems apply payer-specific rules when generating claims and flag situations where a payer policy is likely to result in a downcode or denial so the practice can prepare supporting documentation for an appeal.
